  Abstract Explores whether there is a relationship between staffing and quality indicators in residential care of older people, in both rest homes and continuing-care hospitals. Conducts a longitudinal survey of 18 residential-care rest homes and 16 continuing-care hospitals in the greater Auckland region to explore the relationship between direct-care staffing levels, skill mix and quality indicators. Collects data over a 6-month period on type of staffing, including registered nurses, enrolled nurses, and support workers; quality indicators, including falls, new fractures, pressure ulcers, weight loss, urinary tract infections (UTI), poly-pharmacy or multiple medications, presence of indwelling catheters and use of daily restraints. Uses logistic analysis to analyse the inverse relationship between staffing levels and adverse events, and whether there is an association between staff type and the incidence of the five quality indicators.

  Abstract This thesis reports a mixed methods study, longitudinal in nature, of consenting Age Related Residential Care (ARRC) hospitals in the upper half of the North Island, which was conducted to examine several factors, including AARC hospital efficiency at producing adverse event free days for residents. An interpretativist approach examined what best practice strategies were implemented by the ARRC hospitals that were identified to be most successful at producing adverse event free days for the residents.

  Abstract This was a retrospective study of 100 Auckland people suffering from cancer in their terminal year. It aimed firstly to report the development of limitations in daily living activities and of dependence on help, and secondly to develop a method of assessing the functions of both patients and those who cared for them in such a way that support services could be matched to needs. The observations showed problems of progressive disability and dependence, which varied with the age of the patient and the type of malignancy. They showed clearly the long lasting emotional problems of the careers after the patients' death

  Abstract This research is a study of the experiences of four cultural safety educators in nursing education in Aotearoa, New Zealand. Action research methods assisted the participants to implement change in their practice and gain positive personal involvement in the study. Reflective diaries provided the major tool in this process as participants were able to achieve at least one action research cycle by identifying issues, planning action, observing the action, and reflecting. The findings of the research revealed that the participants not only coped with everyday stressors of teaching but were also required to formulate knowledge for cultural safety. For the Maori participants, their stress was confounded with recruiting and retaining Maori students and macro issues such as commitment to their iwi (tribe). Lack of support to teach cultural safety was identified to be a key theme for all participants. An analysis of this theme revealed that many issues were organisational in nature and out of their control. Action research provided a change strategy for participants to have a sense of control of issues within their practice. Recommendations include the following: support for cultural safety educators to have a dialogue on a regular basis, the introduction of nurse educator programmes, paid leave provisions for educators to conduct and publish research so that a body of knowledge can be developed, and that Maori cultural safety educators be recognised for their professional and cultural strengths so that they do not fall victim to burnout.

  Abstract This paper outlines a phenomenological study carried out to explore the experience of nausea, vomiting and retching associated with pregnancy. These phenomena lack Nursing research and are debilitating symptoms which effect the quality of life for 50-75% of all pregnant women. Madjar ( 1991) indicates that the communication of such experiences can deepen our understanding of human life and coping. It is importance for Nurses to develop an understanding of lived experiences so that they can make more effective interventions. This study explores the essential humanness of life experiences as they are for those who live them. It involved collecting data from those experiencing the phenomena and analysing it. It focuses on the study of phenomena not as separate entities in themselves but as they are perceived as they are experienced. A 'purposeful sample' was required for this study. The aim was to include women who had direct knowledge of the phenomena of nausea, vomiting and retching associated with pregnancy. The study involved in depth interviewing of six women, the interviews were taped and independently transcribed, the transcripts were analysed to determine the meaning of the experience and to identify common themes. The experiences of the women are discussed in relation to what van Manen (1990) describes as four main life world existentials; the lived body, the lived other, lived space and lived time

  Title Involuntary unemployment: A grounded theory analysis of the experience of five nurses Type

  Abstract This study outlines the use of grounded theory strategy to analyse the experience of nurses who become involuntarily unemployed. It then proceeds to develop a theoretical framework that explain the common patterns in this experience. Using the Glasser and Strauss (1967) Grounded Theory approach, empirical observation was undertaken expressly for the purpose of generating insights which may lead to new understanding of the subject of this study. Using two inter-related procedures known as theoretical sampling and constant comparative analysis, data is systematically collected, coordinated and subjected to an ongoing analysis. Theory is then 'grounded ' in the real world. The study involved in depth interviewing of five mid-career nurses who were involuntarily unemployed. The data was transcribed and analysed to yield theoretical concepts and categories that were integrated into propositions to explain common patterns. It will be argued that this experience is a grieving process that is more that just grieving a job loss. It is proposed that there are three phases – 1. Personal devastation due to losses experienced.. 2. A period of healing. 3. Recovery and re-establishment of the 'new' person.

  Abstract High stress and staff turnover in a multi-specialty paediatric area prompted this study that aimed to :1) measure the burnout level of nurses in a multi-specialty paediatric area2) identify and validate causes of stress3) identify new ways of preventing stress on the wardFifty four percent (n=14) of the paediatric nurses completed the questionnaire booklet that included demographic data, the Maslach Burnout Inventory, the Nursing Situations Questionnaire the Hopkins Symptom Checklist-21 the Ways of Coping Checklist and open ended questions about sources of stress and satisfaction at work. Results indicated levelsof burnout and distress comparable with larger studies. Conflict with doctors was the major source of stress followed by workload, inadequate preparation in dealing with the emotional needs of patients and their families and death and dying. Conflict with doctors has not previously been identified as the major source of stress. However workload and death and dying are commonly identified as sources in the literature. Suggestions for further research and the low response rate are discussed

  Abstract This article reviews the recent evidence underpinning the New Zealand Ministry of Health's recommendation to offer free annual influenza vaccination to people aged six months to 64 years who have certain chronic medical conditions (eligible younger people). These results show there is relatively limited research providing evidence underpinning recommendations for influenza vaccination among people aged <65 years. These results show that there is a need to increase nurses' awareness of the rationale behind the New Zealand influenza vaccination policy, that this may in turn increase their willingness to recommend influenza vaccine to more eligible younger people.

  Abstract Advance directives convey consumers' wishes about accepting or refusing future treatment if they become incompetent. There are associated ethical issues for health practitioners and this article considers the features that are relevant to nurses. In New Zealand, consumers have a legal right to use an advance directive that is not limited to life-prolonging care and includes general health procedures. Concerns may arise regarding a consumer's competence and the document's validity. Nurses need to understand their legal and professional obligations to comply with an advance directive. What role does a nurse play and what questions arise for a nurse when advance directives are discussed with consumers? This article considers the cultural dimensions, legal boundaries, consumers' and providers' perspectives, and the medical and nursing positions in New Zealand.
