  Abstract This literature review explores some of the issues related to women's experience of cardiac rehabilitation and demonstrates that women's experience may be different to that of men. Much of the research related to coronary heart disease (CHD) has been performed using either exclusively male populations or such small numbers of women that the results from the women studied were unable to be analysed independently. The author advocates that nurses working within this area of practice require an understanding of women's experience of recovery from a heart attack in order to better meet their needs.

  Abstract The aim of this study was to obtain a 2005 snapshot of the New Zealand rural primary health care workforce, specifically GPs, general practice nurses and community pharmacists. A postal questionnaire was distributed to rural general practice managers, GPs, nurses, community pharmacy managers and pharmacists in November 2005. The self-reported data included information on demographics, country of training, years in practice, business ownership, hours worked including on-call, and intention to leave rural practice.

  Abstract The aim of this paper is to increase neuroscience nurses' awareness of how the head is perceived as sacred by some cultures. This article will outline a definition of culture, discussion around the sanctity of the head for some cultures, the cultural significance of common neuroscience interventions, the use of traditional healing methods, and prayer. Examples will be provided of how nursing interactions and interventions affect some cultures, looking primarily at a Maori and Pacific Island perspective. The focus of this paper is within a New Zealand paediatric setting.

  Abstract This research study examined the role of the public health nurse. Utilising community needs analysis method, 17 key informants and two focus groups were asked questions to determine perceptions of the public health nurse. Findings indicated that participants lacked knowledge regarding the role. Additional findings intimated that participants had difficulty in accessing public health nurse services and that 'knowing the system' was beneficial to receiving needed care. One of the major conclusions of this study was that many facets of care managed by public health nurses were invisible to the communities in which they work. Conclusions suggest that public health nurses need to enhance their service by improving accessibility to services and promoting their service in a more visible manner.

  Abstract This paper traces the emergence of categories of nurse over the last hundred years from the time that the Nurses Registration Act became law in 1901. Insights from the work of Michel Foucault are utilised to show how nurses and nursing have been historically shaped and positioned. It is suggested that the recent endorsement by the Nursing Council of New Zealand of the concept and title of 'nurse practitioner' represents an opportunity for nurses to imagine what might be constructed for their roles.

  Abstract A participatory approach, grounded in both critical social and feminist research, was used in this study of seven women who claim being lesbian as part of their identity. With the objective of providing information to enhance safe care provision for this marginalised group, the study explored factors which hindered or facilitated these women's sense of safety related to health care. The findings indicate that barriers to receiving health care exist for these women. As there is little local or international research on lesbians and healthcare, this work offers a starting place for future New Zealand nursing research involving lesbians. The authors encourage other nurses to explore previously ignored sections of our society in order to strive for safety for all those whom nursing serves.

  Abstract Using thematic analysis, this study explored the phenomenon of effective education for critical care nursing practice by asking: What does effective education for critical care nursing practice mean to nurses currently practising in the specialty? Eighty eight critical care nurses from the South Island provided written descriptions of what effective education for critical care nursing practice meant to them. Descriptive statements were analysed to reveal constituents, themes and essences of meaning. Four core themes of personal quality, practice quality, the learning process and learning needs emerged. Appropriateness or relevance for individual learning needs is further identified as an essential theme within the meaning of effective education for critical care nursing practice. Shared experiences of the phenomenon are made explicit and discussed with reference to education and practice development in the specialty. The study results lend support to education that focuses on individual learning needs, and identifies work based learning as a potential strategy for learning and practice development in critical care nursing.

  Abstract This research aimed to understand the level and scope of practice of the nurse practitioner in Australia and New Zealand further using a capability framework. The original study, from which the present paper was developed, sought to identify competency standards for the extended role of the nurse practitioner in Australia and New Zealand. In doing so the researchers became aware that while competencies described many of the characteristics of the nurse practitioner they did not manage to tell the whole story. In a search of the literature, the concept of capability appeared to provide a potentially useful construct to describe the attributes of the nurse practitioner that went beyond competence. A secondary analysis of data obtained from the interviews with 15 nurse practitioners working in Australia and New Zealand was undertaken. The analysis showed that capability and its dimensions is a useful model for describing the advanced level attributes of nurse practitioners. Thus, nurse practitioners described elements of their practice that involved: using their competences in novel and complex situations as well as the familiar; being creative and innovative; knowing how to learn; having a high level of self-efficacy; and working well in teams. This study suggests dimensions of capability need to be considered in the education and evaluation of nurse practitioners.

  Abstract The aim of this study was to assess intensive care nurses' knowledge of pressure ulcers and the impact of an educational programme on knowledge levels. A knowledge assessment test was developed. A cohort of registered nurses in a tertiary referral hospital in New Zealand had knowledge assessed three times: before an educational programme, within two weeks after the programme, and 20 weeks later. Completion of the educational programme resulted in improved levels of knowledge. Mean scores on the assessment test were 84% at baseline and 89% following the educational programme. The mean baseline score did not differ significantly from the mean 20-week follow-up score of 85%. No association was detected between demographic data and test scores. Content validity and standard setting were verified by using a variety of methods. Levels of knowledge to prevent and manage pressure ulcers were good initially and improved with an educational programme, but soon returned to baseline.

  Abstract The aim of this research is to provide an analysis of the scope of nursing practice and inter-professional role responsibility for ventilatory decision-making in Australian and New Zealand intensive care units (ICU). Self-administered questionnaires were sent to nurse managers of eligible ICUs within Australia and New Zealand. Survey responses were available from 54/180 ICUs. The majority (71%) were located within metropolitan areas and categorised as a tertiary level ICU (50%). The mean number of nurses employed per ICU bed was 4.7 in Australia and 4.2 in New Zealand, with 69% (IQR: 47-80%) of nurses holding a postgraduate specialty qualification. All units reported a 1:1 nurse-to-patient ratio for ventilated patients with 71% reporting a 1:2 nurse-to-patient ratio for non- ventilated patients. Key ventilator decisions, including assessment of weaning and extubation readiness, were reported as predominantly made by nurses and doctors in collaboration. Overall, nurses described high levels of autonomy and influence in ventilator decision-making. Decisions to change ventilator settings, including FiO(2) (91%, 95% CI: 80-97), ventilator rate (65%, 95% CI: 51-77) and pressure support adjustment (57%, 95% CI: 43-71), were made independently by nurses. The authors conclude that the results of the survey suggest that, within the Australian and New Zealand context, nurses participate actively in ventilation and weaning decisions. In addition, they suggest, the results support an association between the education profile and skill-mix of nurses and the level of collaborative practice in ICU.

  Abstract This article presents a reconsideration of partnership between nurse and client as the core of the nursing discipline. It points to the significance of the relational nature of partnership, differentiating its features and form from the prevalent understanding associated with prescriptive interventions to achieve predetermined goals and outcomes. The meaning of partnership is presented within the nursing process where the caring presence of the nurse becomes integral to the health experience of the client as the potential for action. Exemplars provide illustration of this emerging view in practice and research. This is the first of a series of articles written as a partnership between nurse scholars from Iceland, New Zealand and the USA. The series draws on research projects that explored the philosophical, theoretical, ethical and practical nature of nursing practice and its significance for health and healthcare in a world of changing need.
