Abstract |
This phenomenological research project focused on one womans experience following total removal of her larynx and aimed to faithfully capture the essential structure of the phenomenon of voice as it emerged for her. For women in particular gaining a new artifical voice following surgery that is considerably lower sounding, plus the large visible opening in the lower neck is psychologically challenging and can be socially isolating. Most literature relating to laryngectomy and rehabilitation comprises quantitative research with limited attention to womens issues. This could be attributed to the small number of women undergoing this surgery. A review of nursing literature revealed minimal research, with none referring specifically to women. Nor were any studies using entirely qualitative methods identified.This research project used a phenomenological approach informed by the work of Michael Crotty, described as within the parameters of mainstream phenomenology. Based on a traditional common core foundation which is critical, holistic and individualistic, mainstream phenomenology requires achieving co-researchership between researcher and participant while exploring phenomena as objects of authentic human experience.The study began with discussing Crottys stepwise method emphasising the activity of bracketing. The co-researcher was then left to ponder or meditate on her experience following bracketing all her prior understandings and ideas, as if this was the first time she had encountered the phenomenon. Three conversations followed which allowed exploration of the co-researchers written statements. The seven interwoven dimensions that emerged represented the essential structure, meaning and essence of 'what voice is for this woman following total laryngectomy |

Abstract |
It is evident from the prevalence of items in the popular press and incased research by health professional that, over the last two decades, menopause as been discussed more openly. However, medical information is still largely oriented toward menopause as a disease with emphasis on the pharmacological interventions needed to correct the disease. Literature in medical and nursing journals is also predominantly oriented towards menopause as a state of oestrogen, nurse researchers and feminists writers are challenging this viewsThe aim of the research was to discover how women gain knowledge about menopause, and how they make decisions about “managing” their menopause. In the study knowledge was defined as being more than information. It is seen as being more than information. It is seen as understanding derived from synthesis of data about menopause collected from various sources. Eleven women aged 46-55 recruited through a letter in the researcher's local newspaper, were interviewed. The resulting data was analysed by the constant comparison method as used in grounded theory.A descriptive model was developed including the basic social process of “integrating menopause into midlife”. A tertiary level of education and good social support were found to be associated with the women being seekers of knowledge about their menopause. These women, the majority of the participants, revealed themselves as being self-controlling with respect to their menopause. Among the few who had allowed their menopause to be managed by others, if they experienced adverse effects of the treatment, then there was a move toward greater self management.,Generally, nurses were not seen by the participants as possible sources of information. That finding highlights menopause as an area of health education in which nurses have the potential to play a more active role |

Abstract |
There is concern over the standards of nursing practice skill acquisition in undergraduate programs. One of the issues relevant to this is which of the multitude of nursing practice skills to include in an undergraduate program. Previous research by Alavi, Loh and Reilly (1991) has been modified and used in clinical settings in the New Zealand context in order to address this concern. Competency level of the most important skills identified by clinicians was also sought in order to aid development of a skill curriculum for Whitireia Community Polytechnic. There is a strong theoretical support for the preparation of student nurses in skills laboratories prior to clinical learning experience in order to maximise learning. Further directions for study are covered with discussion of the implications for teaching from the research findings |

Abstract |
The aim of this qualitative descriptive study was to explore the experience of new comprehensive nursing graduates as they adapted to working in the acute psychiatric setting. Interviews were conducted with four participants, focussing on their current work experiences and how the philosophical beliefs and values derived from their educational preparation fit with those they encountered within the practice setting. The data were analysed by noting common experiences, values and meanings and identifying the themes as they emerged. The themes were: transition to practice, conflict, contradiction, structural constraints, and the reality of the psychiatric setting. The results of the study confirm the concern that has been voiced by new graduates about the quality and quantity of current orientation programmes. Conflicting values and beliefs concerning the nature of mental health/psychiatric nursing has also become evident. It appears that the graduates' Comprehensive nursing preparation may have contributed to their feelings of unease as they attempted to fit their own values and beliefs about nursing with those of the acute psychiatric setting |

Abstract |
The researchers evaluate and compare the organisation of general practice in the UK and New Zealand. A key aim of the Primary Health Care Strategy is a reduction in health inequalities. Locally, some nurse leaders suggest that changing nurse practice employment from general practitioners to Primary Health Organisations will achieve this aim. The authors take lessons from the UK and suggest that nurses organising themselves into peer groups, remuneration of general practices for the attainment of positive patient outcomes, and a statutory duty of clinical governance, all contributed to the development of practice nurses' roles and expansion of numbers of nurse practitioners in general practice. Nurses have become partners with general practitioners in general practice in the UK, which the authors suggest is a much preferable alternative for some than employment by a Primary Health Organisation. |

The authors present the results of a 2001 New Zealand survey on nurses' perception of staffing, work organisation and outcomes, comparing this with the 2001 International Hospital Outcomes Study (US, Canada, England, Scotland and Germany). The report describes the findings for job dissatisfaction, burnout and the intent to leave, the work climate in hospitals, workforce management, the structure of nurses' work, and quality of care. The authors discuss these findings and their implications for nursing in New Zealand. |

Abstract |
The aim of this study was to explore with a small number of Maori families their experiences of accessing health care when their children were unwell with a respiratory condition. A qualitative research methodology was used in the study. Participating families were among those experiencing an admission to a children's ward between July and December 2003. Four families were interviewed. They discussed in depth their experience of accessing health care for their unwell children. Data were analysed using thematic analysis, and three common themes were evident: family resources, choice of health service provider and parents' feelings of vulnerability. The findings highlight that while socio-economic status plays a large part in determining the ease with which families can access the needed health care, there are other barriers within the health system which also pose difficulties for Maori. |

Abstract |
This paper reports the findings of a preliminary survey of two district health boards, which shows that high workloads and lack of relief staffing appear as the greatest hurdles to mental health nurses participating in practice-based nursing research. A further constraint on their participation is lack of research expertise and experience. Consultative discussions with senior mental health nurses support these conclusions. In this paper, mentoring and flexible research designs are promoted as possible ways of overcoming these barriers. |

This article reports on the New Zealand part of an international study, using agreed study design and instruments, to determine the direct and indirect costs of nursing turnover. These costs also include the systemic costs, estimated by determining the impacts of turnover on patient and nurse outcomes. It presents the findings from the pilot study conducted in six countries to test the availability of costs and suitability of the instrument. Reports the results from a survey of directors of nursing in 20 of the 21 district health boards on turnover and workplace practices. |

The author backgrounds the development of the role of an innovative Nurse Researcher (Emergency Medicine) role at Christchurch Hospital. She describes the emergency department and the factors leading to the creation of the role. Specific nursing research projects are reviewed, and the nature of nursing in relation to research is discussed. The author argues that the nurse researcher is integral to the expansion of evidence-based nursing, and that the role of Clinical Nurse Researcher in the emergency department has resulted in a higher profile for research, and the gradual integration of research as a clinical skill with direct practical relevance. |
