  Abstract This article discusses the important role that preconceptions play in the process by which students learn to be nurses. The importance of preconceptions emerged from the analysis of data in a grounded theory study that sought to gain a greater understanding of how undergraduate student nurses in New Zealand experienced and responded to differences they perceived between the theory and the practice of nursing. It became clear that the preconceptions each student nurse held about the nature of nurses and nursing care were the standards against which the worth of the formal, practical and personal theories to which students were exposed during their nursing degree was evaluated. It was clear that preconceptions functioned as the mediator between the intentions of nursing education and the learning that eventuated for each student from practicum experiences. The implications for nursing education, for which preconceptions are not generally highly valued as a basis for learning about professional nursing, are that the individual experience and personal characteristics of each student receive significant focus when a nursing programme is planned. This means that the orthodox principles that underpin the design of nursing curricula should be reviewed and an overtly constructivist perspective adopted for nursing education for which the prior experiences of the student are the starting point.

  Abstract This paper reports the results of a survey to determine how much genetics is taught in the bioscience component of the three-year Bachelor of Nursing degree offered by 16 tertiary education institutes in New Zealand. A questionnaire was mailed to the bioscience lecturers seeking information on the bioscience and genetics content of current programmes. They were also asked to indicate their perception of the impact and relevance of new genetic knowledge on health care and nursing education. Results indicated that on average 250-350 hours are devoted to the teaching of biosciences. Less than 10 hours are devoted to genetics at 66% of institutes, one institute did not teach any aspect of genetics. None of the institutes taught more than 20 hours of genetics in the programme, although 47% of lecturers said they would like to teach more genetics if there were more time available in the curriculum. Lecturers teaching bioscience to Bachelor of Nursing students are aware of the importance of genetics in health care and to nursing in particular, and the majority are of the opinion that more genetics should be included in undergraduate programmes, however 'curriculum crowding' is a problem.

  Abstract This paper explores the relationships within teaching, learning and practice development in critical care nursing and questions the popular assumption that 'post graduate (Master's level) education fits all'. The need for critical care nurses to apply advanced knowledge and technical skills to complex and dynamic practice situations necessitates the development of critical thinking and a problem-solving approach to clinical practice that can be fostered through education and experience. Discussion focuses on the successful development and implementation of graduate level education for critical care nurses in the South Island of New Zealand and how this development is challenging existing approaches to the provision and evaluation of formal critical care education in New Zealand.

  Abstract The educational framework used to teach pharmacology to nurses by one university in New Zealand is presented, along with early findings on the effectiveness of this approach. Nurse prescribing is relatively new in New Zealand and is related to the expanding roles and opportunities for nurses in health care. Opposition to nurse prescribing in New Zealand has been marked and often this has been linked to concerns over patient safety with the implication that nurses could not be adequately prepared for safe prescribing.

The weaving of pharmacology through the three-year undergraduate curriculum is described, showing the development of a pharmacology curricula thread. The significance of supporting curricula content in areas such as communication skills, law and ethics, as well as sound biological science and physiology knowledge are highlighted.

  Abstract This paper describes how a new framework for clinical nursing education was introduced at Counties Manukau District Health Board. The project was initiated in response to the significant legislative and post registration nursing education changes within New Zealand. The journey of change has been a significant undertaking, and has required clear management, strong leadership, perseverance and understanding of the organisation's culture. The approach taken to managing the change had four stages, and reflects various change management models. The first stage, the identification process, identified the impetus for change. Creating the vision is the second stage and identified what the change would look like within the organisation. To ensure success and to guide the process of change a realistic and sustainable vision was developed. Implementing the vision was the third stage, and discusses the communication and pilot phase of implementing the nursing education framework. Stage four, embedding the vision, explores the process and experiences of changing an education culture and embedding the vision into an organisation. The paper concludes by discussing the importance of implementing robust, consistent, strategic and collaborative processes that reflect and evaluate best educational nursing practice.

  Abstract M-Learning compliments and supports E-Learning and incorporates technologies such as the use of mobile phones, PDAs and pod casting. A sub function of M-Learning is M-Support which is the provision of support for the mobile student. This paper describes a pilot project using short message service – mobile technology -to M-support nursing students in primary health care clinical placements. Forty one students took part in a mixed methods study to assess the project against a pre determined set of criteria. The results indicated that students did feel supported by the messages, cost was not prohibitive, flexibility in communication was appreciated, and the acceptance was high.

  Abstract This thesis presents the job of psychiatric nursing from the nurse's point of view, as derived from the author's personal experience and from interviews with thirty five other staff within a particular psychiatric institution.The first part of the thesis is reconstructed narrative account of an afternoon and a day shift in an acute admission ward. In the second part, the basic situation on the ward and some of the exigencies of nurse-patient and nurse-staff relations are discussed from structuralist and strategic conduct perspectives.The discussion that follows Anthony Giddens' (1976, 1979, 1984) conceptual framework of power, legitimation and signification, with particular attention to the strategic implications of ward routines, nursing practices, and interpersonal relations, as well as the duality of clinical and moralistic interpretive themes. The third part of the thesis 'the nurse's progress' over time. Characteristic changes in understanding and awareness take place with the movement from the 'backwards' to the 'acute' area and from the student to staff nurse. This is portrayed as a 'moral career' analogous to that suggested by Goffman (1968) for psychiatric patients; marked by 'happenings' that generate revised conceptions of self and others, and including those experiences of duality and contradiction discussed in part Two. While acknowledging the diversity of nurses' attitudes and approaches, with variations according to individual temperament, past experiences and the current setting, the suggestion is made of a common and distinctive 'meta-awareness' that develops with the fob

  Abstract Nursing is increasingly using holistic interventions. Some research validating these interventions has been conducted. However there was no nursing research for these interventions with women with breast cancer. This research explored the experience of one womean who was diagnosed with breast cancer and who actively chose to have these interventions as an adjunct to the medical treatment.It was a single subject phenomenological study, effectively being a pilot study. Data was collected through unstructured interview, participant observation and review of the nursing notes. Interpretative analysis techniques were used to obtain the results. Three themes of the experience of the subject were identified as:1. maintenance of the locus of control with the subject,2. the choice of a health professional with philosophical base similar to that of the subject and3. maintenance of a focus on healthy living.These interventions were perceived as beneficial by this subject. This research demonstrated that further study on the effectiveness of holistic nursing interventions with women being treated for breast cancer is indicated

  Abstract This is an action study where 298 patient lifts were observed and analysed according to environmental patient and nurse activity factors, frequency of different types of lifts were also recorded and Nursing lifting loads estimated. Findings showed lack of planning in preparation for lifts, a tendency for Nurses to perform lifts without help and lifting loads and lift types which put Nurses at increased risk of injury. Suggestions are made for giving patient lifting a lift. These require action at individual nurse and organisational levels

  Abstract In this article, the author will discuss the way in which the task of finding out whether staff levels were adequate in a long term ward was approached. Figures were collated using a patient dependency rating (described below) in use at the time of the article, estimated nursing hours per patient and staff numbers per day; the 3 sets of figures were kept over a 28 day period. The results gained were compared to results from articles and reports perused by the author and conclusions reached as to whether the staff levels were adequate or not. Following this, implications of lack of staff and possible solutions were discussed
