Abstract: We are about to implement a nurse-based critical care follow-up service and used a follow-up study of tracheostomy to determine the feasibility of such a service. Nine to 27 months median 14 after intensive care admission we attempted to follow-up 116 of our most severely ill survivors. Twenty six could not be found, 11 declined follow-up. A questionnaire, interview and limited examination were used to determine health status. Ten patients unable to come to a clinic were seen at home. Interview time varied between patients but 79 patients took 100 nurse-hours. Ninety percent of patients co-operated with respiratory function testing. Pulse oximetry identified one with severe chronic respiratory failure. In this highly selected group of patients with on going problems (perceived poor health, dependency, disability, changes in appearance, deterioration in personal relationships, poor sleep, pain, altered diet, unaddressed chronic health problems, poor access to health services, and continuing prescription medication) were common, and addressed to some extent by appropriate referral. Approximately 20% of patients reported improvement in their health and personal relationships after critical illness and half of the smokers had given up. Factors contributing to follow-up difficulty included distance, delay in initial contact, change of address (255 of this group), disability and poverty. A nurse-run critical care follow-up clinic is feasible, time consuming but productive. A register of current addresses and phone numbers is recommended. The optimal time for follow-up will vary with patients conditions but early follow-up with protocol- based lines of referral is recommended to reduce persistent health problems

Abstract: A nurses working party was formed by the primary health care SSDG in 1979 to investigate the feasibility of integrated community based nursing services. A six month trial was held in a mainly urban area (population 5637) testing these ideas in 1980. Using the existing time of the four nursing services in the area a team approach was used with all nurses being responsible to a coordinating nurse. The present role of the public nurse and district nurse was integrated – this new nurse was called a community health nurse. These two nurses worked from a base located in te trial area. Liaison and coordination were established with the Plunket and practice nurse. The results of the trial enabled the primary health care SSDG to prepare a plan on community based nursing services

Abstract: In 1991 a new labor relations regime was introduced which overturned a 100 year old pattern of 'historic compromise' between capital and labor. In a labor market structured by gender and race, this major change in bargaining arrangements has already widened the pay gap between men's and women's average earnings and reduced union coverage, particularly among women workers in low paid clerical, sales and service work. This study, documenting recent feminist struggle in the area of labor relations, provides a first look at the collective organisation of women under two different labor relations regimes.In the 1980s a particular conjunction of occupational unionism and feminism in New Zealand facilitated some significant improvements in the situation of women in paid employment. The thesis examines feminist strategies which led to a Working Women's Charter adopted by unions, an increase in women holding office in unions, complaints procedures for sexual harassment, standing committees to represent women and Maori in the union movement, and legislation to implement equal employment opportunity programmes and equal pay for work of equal value. It looks at how the institution-alisation of bargaining by occupation supported industrially weak workers and underpinned the unionisation of women, which occupational unions and women's own strategies of organisation provided the autonomous 'political space' to organise around issues specifically relevant to women.At the core of the thesis are three case studies of unions representing three of the occupations in which women are concentrated: clerical work, nursing and cleaning. It examines commonalities and contrasts in the industrial situations covered by these unions, and differences and similarities in the strategies they adopted. The focus of research, conducted between late 1990 and early1993, was the views of officials of these unions in the context of radical change in the regulation of wage bargaining. Particular attention was given to the way issues relating to women workers were prioritised in unions led by women or by men.These case studies are contextualised in chapters examining the position of women in the labor market, feminist organisation within the union movement, and corporatist change in labor relations legislation. In tracing the development of feminist unionism in the 1980s, the thesis considers the strategies of the NZ Council of Trade Unions and looks at what the radical change in bargaining structure will mean for collective organisation by women in paid employment

Abstract: This study explores the phenomenon of a personal exploratory field visit to HIV programmes in Malawi and how that informs the author's future plans to work cross-culturally with HIV. He uses hermeneutic phenomenology with the guidance of Heidegger and Gadamer, and draw on Ackermann, Hill, Maluleke, Moltmann, and Thielicke for theological direction. This study analyses how personal formation takes place and how the meaning of that experience can inform future cross-cultural interaction. The data of this study is drawn from a range of people interviewing 'me'. This includes a pre and post interview in relation to the author's three week exploratory visit to Malawi, and recorded daily reflections during the visit. Upon return he was interviewed about the experience by ten people from the following areas: nursing, counselling, development, theology, business, medicine, clergy, an Expatriate Malawian, and a women working from a Maori paradigm. These interviews focused on the author's experience with questions framed from the interviewer's specialty area. The transcripts become further data for this study. The findings of this thesis suggest that people wishing to work cross-culturally need to understand their motivation for their work, and understand who they are before entering a foreign land. This transformative journey also needs to continue as part of the process of working with people because we can only be effective with change if we are listening and hearing the other's perspective. It is in being open to this difference between persons that we continue to find ourselves. While perhaps we have a tendency to want to make everybody like us, we can only grow into our full potential in relationship with truly different others. Tensions experienced demonstrate that there is a complex need to understand how the context controls how HIV is perceived. This requires uncovering some of the deeper issues of HIV and culture, and knowing how to conceptualise these in both positive and informative ways. This thesis asks four key questions for the global-co-worker to work through before embarking on cross-cultural mission: 1. How do you know you should go?; 2. How are you going to make a difference?; 3. Who are you going to be?; and 4. What will sustain your involvement? The author's own experience has drawn me into a deeper awareness of the need for a vital connectedness of faith, hope and love underpinning the everydayness of such an experience.

Abstract: This paper describes a survey undertaken with postgraduate nursing students in a university-based school of nursing in 2002 to establish their access to and use of computers and information technology for study. Whilst there was minimal flexibility and use of technology to support student learning for postgraduate nurses in the school, the university proposed increasing flexibility across all courses. This is in part a response to the increased internationalisation of education and developments in technology affecting programme design, delivery and support that can benefit teachers and students. The author notes that the findings of this survey form a basis for planning the introduction of flexible learning. Results indicated that not all students have convenient access to technology for study purposes, nor are they at the same level in terms of using technology.

Abstract: This article describes a study conducted on the internal medicine, general surgical, and vascular wards of a large metropolitan hospital to assess the impact of a networked monitoring system and portable patient monitors. This pilot study was developed to address the needs of hospital patients who require continuous non-invasive vital signs monitoring (including heart rate, non-invasive blood pressure, pulse oximetry, cardiac waveform monitoring) with the addition of surveillance from a cardiac intensive care area. Data were collected from 114 patients over a three-month period to identify a patient group that could be managed appropriately under the new system and to determine the effect that flexible monitoring had on patient care management. Findings include identification of a specific patient group that can be managed successfully outside the cardiac intensive care area using this system. Other findings suggest a way to improve the management of patient monitoring in the general ward areas.

The research project sought to describe the flight nurses perceptions about the factors that influence clinical decision making in their flight nursing practice, using a descriptive survey methodology. Themes emerged as factors which participants perceived influenced clinical decision making in their flight nursing role and in the aeromedical role. These themes included pre-flight preparation, patient status, experience and education of the nurse, and the challenges associated with the physical and atmospheric environments.
